Shabrez Tariq, MD, CLCP, CPLCP, LCP-C

At PMR Life Care Planner, we are proud to be led by Dr. Shabrez Tariq, a distinguished physician renowned for his expertise in pain management, physical medicine, and rehabilitation. With a Bachelor of Medicine and Bachelor of Surgery (M.B.B.S) degree from JN Medical College in India and a rigorous residency in general surgery, Dr. Tariq has cultivated invaluable experience across various surgical disciplines. His passion for holistic patient care guided him to specialize in Physical Medicine and Rehabilitation at Baylor College of Medicine in Houston, Texas, followed by a fellowship in interventional pain and spine at Pain and Spine Consultants in Nashville, Tennessee.

As the Medical Director of Houston Spine and Joint Pain Consultants since 2011, Dr. Tariq has dedicated himself to serving the Houston community, focusing on spinal cord injury, traumatic brain injury, amputation, and chronic pain management. His commitment to excellence is underscored by his board certifications in Pain Medicine and Interventional Pain Practice, as well as prestigious designations as a Certified Life Care Planner (CLCP) from the International Commission on Health Care Certification (ICHCC) and a Certified Physician Life Care Planner (CPLCP) from the American Academy of Physician Life Care Planners (AAPLCP).

Dr. Tariq’s clinical expertise is widely recognized, and his compassionate approach to patient care has earned him the respect and gratitude of his patients. Additionally, he serves as an expert witness, where his profound knowledge and articulate communication skills significantly enhance legal proceedings. His ability to convey complex medical information in an accessible manner makes him a valuable asset in personal injury cases, ensuring that clients receive the advocacy they deserve. Candace Koney, MD, MHA, LCP-C, CHLCP

Dr. Candace Koney is a highly qualified Life Care Planner with extensive experience in health and hospital administration, anesthesiology, and occupational medicine. Raised in Shaker Heights, Ohio, Dr. Koney completed her undergraduate studies at Denison University before earning her graduate degree in Health and Hospital Administration and her Medical degree from The Ohio State University.

With a residency completed in Anesthesiology at the University of Florida in 2003, Dr. Koney practiced as an Anesthesiologist for seven years at Anesthesia Associates in New Port Richey, Florida, where she became a partner within the practice. She expanded her expertise by completing a residency in Occupational Medicine at The University of Pennsylvania in 2018. Subsequently, Dr. Koney served as Medical Director for four Concentra clinics in Tampa, Florida, and played a vital role as a consultant and Medical Lead for the COVID response team at Unilever, North America.

Currently, Dr. Koney is the Chief Medical Officer at Main Street Medical and Acupuncture, a community health clinic that specializes in Occupational Medicine, Urgent Care, and Acupuncture. She is actively involved in the American College of Occupational and Environmental Medicine (ACOEM), having servedas Delegate in the Fitness and Disability section, as well as Director of Education. Additionally, Dr. Koney is a board member of the Florida Association of Occupational and Environmental Medicine.

Dr. Koney completed her Life Care Planning training through FIG Education in 2019, earning the credential Life Care Planning-Certified (LCP-C). Most recently, in January 2025, she obtained the Certified Health Professional Life Care Planner (CHLCP) certification. With her comprehensive background and commitment to improving patient care, Dr. Koney is dedicated to providing exceptional life care planning services that cater to the unique needs of individuals.

Sheeba Farhat, APRN, LCP-C

Sheeba Farhat is a highly skilled healthcare professional with over 20 years of diverse nursing experience, primarily focused on critical care and life care planning. With a strong foundation in both patient care and leadership, Sheeba has built a reputation as a compassionate advocate for her patients, ensuring they receive the highest standard of care throughout their healthcare journey.

Sheeba earned her Master of Science in Nursing from Barry University, specializing in Adult Gerontology Acute Care. As a board-certified Acute Care Nurse Practitioner, Sheeba has spent the past eight years working in an Intensive Care Unit setting, where she has honed her clinical expertise in managing complex medical conditions. Her extensive experience also includes roles as a Nursing Supervisor and Quality Improvement Specialist, allowing her to implement evidence-based practices and enhance patient outcomes in various healthcare environments.

In her current role as a Nurse Life Care Planner, Sheeba combines her clinical knowledge and experience in patient care to create detailed life care plans that support individuals with chronic health conditions or significant disabilities.
